Handouts

Print, Share or Email

A referral lands better when the family knows why they are being sent and what happens next. These handouts explain it in plain language, so the conversation you started in your office carries on at home. Print as many as you like, or email the PDF straight to a family.

  • What I've noticed in class

A note a teacher can tick and send home: the classroom signs that the eyes are working harder than they should, even when a school screening was passed.

  • Classroom accommodations for visual difficulties (504 / IEP)

A fill-in sheet of classroom accommodations for a student whose visual difficulty makes sustained reading, copying and near work tiring.

  • While you wait: home and classroom tips

Simple changes that lighten the load on tired eyes at home and at school until the evaluation. They support the child; they do not treat the problem.

  • What to expect at your evaluation

A one-page walk through the visit for the family: booking and forms, the evaluation itself, results the same day, the written report, what to bring and how payment works.
