Convergence Insufficiency — One of the Most Missed Vision Problems

What convergence insufficiency is

To read or look at anything up close, your eyes have to turn slightly inward — to converge — and stay there comfortably. With convergence insufficiency (CI), the eyes have trouble doing this. They tend to drift outward at near, and the visual system has to work overtime to keep a single, clear image. The result is that reading and screen work become uncomfortable, tiring, or even produce double vision.

CI is common and very treatable — yet it’s one of the most frequently missed vision problems, because a standard eye exam centered on distance clarity may never test it.

Symptoms to recognize

  • Headaches or eye strain with reading or screens
  • Words that blur, swim, or appear to double on the page
  • Losing place, skipping lines, or re-reading
  • Difficulty concentrating during near work
  • Avoiding reading and other close tasks
  • Sleepiness or fatigue when reading
  • Closing or covering one eye to read

Because these symptoms cluster around near work, CI is often mistaken for an attention problem or simple reluctance to read.

What the research shows

Convergence insufficiency has been studied rigorously. The Convergence Insufficiency Treatment Trial (CITT), a randomized clinical trial funded by the National Eye Institute, found that office-based vision therapy combined with home reinforcement was the most effective treatment studied for children with symptomatic CI — more effective than home-based or pencil-only approaches.

How it’s treated

Treatment typically involves a course of vision therapy that retrains the eyes to converge and sustain alignment comfortably, supported by short home activities. Many patients notice that reading becomes dramatically less effortful as the skill develops.

What to do next

If reading comes with headaches, blur, or fatigue — for a child or an adult — convergence insufficiency is worth ruling in or out. A comprehensive evaluation tests for it specifically. Reach out and we’ll take a careful look.

Frequently Asked Questions

Common Questions About This Topic

Can convergence insufficiency be cured?

It responds very well to treatment. Research has shown office-based vision therapy with home reinforcement to be the most effective approach studied for symptomatic convergence insufficiency.

Why was my child's CI missed at the eye doctor?

A routine exam often focuses on distance clarity and eye health. Convergence is a near-vision teaming skill, so it can be missed unless it's specifically tested.
